Μελέτη της επίδρασης των αντιεπιληπτικών φαρμάκων στη σωματική αύξηση, τις ορμόνες λεπτίνη και γκρελίνη και τη συσχέτισή τους με τη θυρεοειδική λειτουργία σε παιδιά με επιληψία ; The effect of antiepileptic drugs (AEDs) on growth, leptin , ghrelin and their relation to thyroid function in epileptic children

Introduction: Epilepsy and AEDs affect growth and hormonal balance with unclear pathogenicmechanisms.Aims: To evaluate the effect of oxcarbazepine (OXC) and valproate (VPA) on growth, thyroid function,peptide hormones and lipid profile.Materials-Methods: 59 children, aged 3.7-15.4yrs, 26 on OXC and 33 on VPA monotherapy wereevaluated before, after 8 and 18 months of treatment. Data were analyzed using SPSS 17.0. P<0.05was considered significant.Results: At 8th month, the percentage of overweight/obese children climbed from 23% to 38.5 %(OXC), and from 33.3 % to 54.5 % (VPA). The Δ-SDS-BW, Δ-SDS-BMI and TSH levels were increasedsignificantly, while FT4 levels were decreased in both groups. A significant increase of LDL-cholesterol and γGT levels was demonstrated only in OXC group. Lipoprotein (a), [Lp(a)], was increased in bothgroups. Further increases in obesity rates were noted only in the VPA group at 18 months. The use ofOXC and VPA had no significant effect on Δ-SDS-Height and, the leptin, adiponectin and total-activeghrelin levels. Compared to controls epileptic children had significantly higher leptin and lowertotal/active ghrelin levels.Conclusions: Epileptic children on OXC or VPA monotherapy are at increased risk foroverweight/obesity, hypothyroidism and dyslipidemia. Furthermore, epilepsy may be associated withhigh plasma leptin and low plasma ghrelin levels. ; Εισαγωγή: Τα αντιεπιληπτικά και η επιληπτική νόσος επιδρούν στη σωματική αύξηση και στο ορμονικό σύστημα των παιδιών, με μη πλήρως διευκρινισμένους μηχανισμούς.Σκοπός: Μελέτη της επίδρασης της οξκαρβαζεπίνης (OXC) και του βαλπροϊκού (VPA) σε: σωματική αύξηση, θυρεοειδική λειτουργία, πεπτιδικές ορμόνες, λιπίδια αίματος. Υλικό-Μεθοδολογία: 59 παιδιά, ηλικίας 3.7-15.4 χρόνων, 26 σε μονοθεραπεία με OXC και 33 με VPAαξιολογήθηκαν προ αγωγής, 8ο και 18ο μήνα χορήγησης.
