Successful Treatment of Valproic Acid and Carbamazepine Overdose With Hemoperfusion.

Anticonvulsant agents are used in many areas of medicine. Labeled uses of Carbamazepine (CZP) include the treatment of partial seizures, generalized tonic-dome seizures, mixed seizures and pain relief in trigeminal neuralgia. Unlabeled uses include treatment of bipolar disorder, resistant schizophrenia, alcohol withdrawal, restless leg syndrome, psychotic behavior associated with dementia and post-traumatic stress disorder (Lacy, 2001). labeled uses of valproic acid (VPA) are mania associated with bipolar disorder, migraine prophylaxis, treatment of complex partial seizures, and treatment of absence seizures. Unlabeled uses of VPA include treatment of behavior disorders in Alzheimer's disease (Lacy, 2001). Overdose with these agents can cause nausea, vomiting, hepatotoxicity and eventually lead to coma and death. In this article we present two cases of anti-convulsant overdose and the use of hemoperfusion, a dialysis-like therapy, in the treatment. We present the case of an 13-year old, white male who was found after the ingestion of valproic acid and the use of bemoperfusion in the treatment. We also present the case of a 32-year-old, white female who attempted suicide with carbamazepine and the role of hemoperfusion in her treatment. We conclude with a discussion of treatment of poisonings and make the recommendation that for CZP and VPA overdose, hemoperfusion it the treatment of choice. [ABSTRACT FROM AUTHOR]
