The nicotinism is the cause of 4 million death in the world including more than 70% in the developing countries. The purpose of our study was to determine the prevalence of the nicotinism and to identify the factors which influence it in school in Bamako. We carried out a transverse investigation in April 2009, in 8 selected schools in a random way in a commune of Bamako, with a sample of 328 pupils of more than 11 years. The investigation concerned 328 pupils including 72,3% boys. The age of the pupils varied between 12 and 23 years. The age bracket 12- l5ans was represented with 66,2%. It arises that 47,6% of the pupils smoked at least once of the cigarette. We found that 14% of the pupils (46/328) were regular smokers including 100% boys. Among these nicotinic, 37% smoked more than 5 cigarettes per day and 52% lasted one consumption higher than 12 months. The factors supporting the nicotinism were: father smoker (22,56%), family entourage smoker (56,4%), teaching smokers (52,7%). The main motivations which pushed the pupils to be smoked were promiscuity (63%) the pleasure (22%) and the snobbery (13%). If 85,9% of the pupils knew the harmfulness of the tobacco, 38,7% learned it at the school but 76,5% were unaware of the existence of a law on the sale and the consumption of the tobacco in Mali. The prevalence of the nicotinism at the pupils was of 14% in an inciting environment. We recommend to intensify the communication for the behavioral change and to apply the law to the tobacco.
