Individualised care -- comparison of patients' and nurses' assessments. (English)

Individualised care has been studied from both the patients' and nurses' point of views in different healthcare contexts. However, comparisons between patients' and nurses' assessments at the same time have not been reported. The aim of this study was to compare patients' and nurses' perceptions on individualised nursing care. Data were collected from two hospital districts area, seven hospitals and fourteen general surgical units using questionnaires including the Individualised Care Scale and background variables. In consecutive order 25 discharging patients from each unit (N = 357, n = 291, response rate 82 %) were recruited during the data collection period 9.9.-30.9.2009. Total sample of nurses (N = 360, n = 283, 79 %) were recruited. Data were analysed using descriptive statistics and t-test for comparison. Patients perceived that nurses have supported their individuality through nursing activities to some extent, most in the clinical situation and least in personal life situation. Nurses perceived that they have supported patient individuality well, and their assessments compared to patients were better in all content areas. Both the patients and nurses regarded that the care was individualised, and these assessments were similar and corresponded well, except in the clinical situation, where nurses' assessments were more positive. Yksilöllistä hoitoa on tutkittu aiemmin sekä potilaiden että hoitajien osalta erilaisissa hoitotyön ympäristöissä, mutta samanaikaisia arviointeja yksilöllisestä hoidosta ei ole raportoitu. Tutkitnuksen tarkoituksena oli verrata potilaiden ja hoitajien näkemyksiä yksilöllisestä hoidosta. Aineisto kerättiin kyselylomakkeilla kahden sairaanhoitopiirin seitsemän sairaalan yleiskirurgisilta vuodeosastoilta (n=14). Kyselylomake sisälsi Yksilöllinen hoito -mittarin sekä taustamuuttujatiedot. Jokaista vuodeosastoa kohden jaettiin tutkimusajankohtana 9.9.-30.9.2009 kyselylomake järjestyksessä jokaiselta osastolta kotiutuvalle 25 potilaalle (n = 291, vastausosuus 82 %). Otantamenetelmä oli hoitajien osalta kokonaisotanta (n = 283, vastausosuus 79 %), Aineisto analysoitiin kuvailevan tilastotieteen menetelmin ja vertailut tehtiin kahden otoksen t-testillä. Potilaiden mielestä heidän yksilöllisyyt-tään on tuettu jonkin verran hoitotyön keinoin, parhaiten kliinisen tilanteen ja heikoiten henki-lökohtaisen elämäntilanteen osalta. Hoitajat arvioivat tukevansa potilaiden yksilöllisyyttä hyvin, mutta heidän arvionsa olivat potilaiden arvioita paremmat kaikissa arvioiduissa osissa. Sekä potilaat että hoitajat arvioivat, että hoito oli yksilöllistä. Arvioinnit olivat hyvin samanlaiset ja näkemykset erosivat toisistaan ainoastaan kliinisen tilanteen osalta hoitajien arvion ollessa parempi.
