Working with chronic and relentless self-hatred, self-harm and existential shame: a clinical study and reflections.

This paper is the first of a two-part series which explores some of the theoretical and experiential reference points that have emerged in my work with people whose relationship to their body and/or sense of self is dominated by self-hatred and (what Hultberg describes as) existential shame. The first paper focuses on self-hatred and the second paper focuses on shame. This first paper is structured around vignettes taken from a 14-year analysis with a woman who was bulimic, self-harmed and repeatedly described herself as 'feeling like a piece of shit'. It draws together elements of Jung's concepts of the complex and symbol, and Laplanche's enigmatic signifier to focus on experiences of 'inner otherness' around which we are unconsciously organized. It also brings Jung's understanding that emotion is the chief source of consciousness into conversation with Laplanche's approach to the transference which is that by being aware that they do not 'know', the analyst provides a 'hollow' in which the patient's analytic process can evolve. These combinations of ideas are linked speculatively to emerging understandings of the neuroscience of perception and throughout the paper clinical material is used to illustrate these discussions. El presente trabajo es el primero de una serie de dos partes, que explora algunos puntos de referencia teóricos y experienciales que han emergido en mi trabajo con personas cuya relación con su cuerpo y /o sentido de sí misma es dominado por la autodenigración y por (aquello que Hultberg describe como) la vergüenza existencial. El primer ensayo focaliza en el sentimiento de autodenigración y el segundo en la vergüenza. El presente ensayo se estructura alrededor de viñetas clínicas tomadas a partir de un análisis de catorce años con una mujer bulímica, que se auto infligía daño y se describía a sí misma repetidamente 'como un pedazo de mierda'. Integra los conceptos de Jung de complejo y símbolo, y el 'significante enigmático' de Laplanche, para dar cuenta de aquellas experiencias de 'otredad interna' alrededor de las cuales estamos inconscientemente organizados. También introduce una conversación entre la comprensión de Jung respecto de la emoción como fuente principal de devenir conciente, y la noción de la transferencia de Laplanche, la cual propone que a partir del no 'saber' del analista, éste provee un 'espacio vacío' en el cual puede desplegarse el proceso analítico del paciente. Esta combinación de ideas se enlaza especulativamente con comprensiones emergentes de las neurociencias de la percepción. Translations of abstract Cet article est le premier de deux articles qui explorent quelques points de référence théoriques et expérimentaux qui ont émergé dans mon travail avec des personnes dont la relation à leur corps et/ou au sens du soi est dominée par la haine de soi et par ce qu'Hultberg décrit en tant que honte existentielle. Le premier article porte sur la haine de soi, le second se concentrera sur la honte. Ce premier article est construit autour de vignettes provenant d'une analyse de quatorze années avec une femme qui était boulimique, qui se mutilait, et qui parlait souvent d'elle comme « se ressentant comme un morceau de merde ». L'article établit des liens entre les concepts de complexe et de symbole chez Jung et le « signifiant énigmatique » chez Laplanche, afin de se concentrer sur les expériences d' « altérité intérieure » autour desquelles nous sommes inconsciemment organisés. L'article amène aussi un échange entre la compréhension par Jung de l'émotion en tant que principale source de la conscience et l'approche du transfert par Laplanche. Dans cette approche, l'analyste, par le fait qu'il a conscience qu'il ne « sait » pas, offre à son patient un « creux » dans lequel le processus analytique du patient peut évoluer. Ces combinaisons d'idées sont reliées de manière spéculative à la compréhension émergente des neurosciences de la perception, et tout au long de l'article, du matériel clinique est utilisé pour illustrer les discussions. Dieser Text ist der erste einer zweiteiligen Serie, die sich mit einigen der theoretischen und empirischen Bezugspunkte beschäftigt, die in meiner Arbeit mit Menschen aufgetaucht sind, deren Beziehung zu ihrem Körper und / oder zum Selbst von Selbsthaß und von dem, was Hultberg als existentielle Scham beschreibt, dominiert ist. Der erste Teil fokussiert den Selbsthaß und der zweite wird die Scham in den Mittelpunkt stellen. Dieser erste Text ist um Vignetten der vierzehnjährigen Analyse einer Frau herum strukturiert, die bulimisch sowie selbstverletzend agierte und sich wiederholt umschrieb mit: 'Ich fühle mich wie ein Stück Scheiße'. Er führt Elemente von Jungs Konzepten des Komplexes und des Symbols und Laplanches 'rätselhaftem Signifikanten' zusammen um die Erfahrung des 'inneren Andersseins' herauszustellen, um die herum wir unbewußt organisiert sind. Darüber hinaus stellt der Text eine Korrespondenz her zwischen Jungs Ansicht, daß Emotionen die Hauptquelle des Bewußtseins seien, zu Laplanches Auffassung der Übertragung, die darin besteht, daß der Analytiker durch das Wissen, daß er nicht 'weiß' einen 'Hohlraum' bereitstellt, in dem sich der analytische Prozeß des Patienten entfalten kann. Diese Kombinationen von Ideen werden spekulativ mit sich entwickelnden Erkenntnissen der Neurowissenschaften über die Wahrnehmung verbunden. Questo scritto è il primo di una serie in due parti che ha origine dal mio lavoro con persone la cui relazione con il proprio corpo e/o con un senso di sé era dominato da un odio per se stessi e (ciò che Hultberg descrive come) da una vergogna esistenziale. Il primo lavoro si struttura intorno a vignettes tratte da una analisi di 14 anni con una donna che era bulimica, odiava se stessa e ripetutamente si descriveva come 'sentirsi come un pezzo di merda. Ciò mette insieme elementi del concetti Junghiani del complesso e del simbolo e 'l'enigmatico significato di Laplanche' per focalizzarsi intorno a esperienze di 'interne alterità' intorno alle quali noi siamo inconsciamente organizzati. Ciò porta anche alla comprensione di Jung che , conversando con l'approccio di Laplanche nei confronti del tranfert, sostenne che l'emozione è la principale sorgente della consapevolezza e che l'analista, essendo consapevole che loro 'non sanno', fornisce 'un vuoto' nel quale il processo analitico può evolvere. Tali combinazioni di idee sono speculativamente legate alle emergenti comprensioni delle neuroscienze della percezione e il materiale clinico scritto viene utilizzato per illustrare la discussione. Данная статья - первая из серии из двух статей, которая исследует некоторые теоретические и эмпирические опорные точки, появившиеся в работе с людьми, чье отношение к собственному телу и/или чувству себя пребывало в состоянии ненависти к себе и того, что Хатлберг описывает как экзистенциальный стыд. Первая статья композиционно выстроена вокруг виньеток из 14-летней работы с женщиной, страдавшей булимией, наносившей себе вред и постоянно твердившей, что она «чувствует себя, как кусок дерьма». Статья сводит воедино элементы концепций Юнга о комплексе и символе и «загадочного означающего» Лапланша, фокусируясь на переживаниях «внутренней инаковости», вокруг которых мы бессознательно организуемся. Понимание Юнгом эмоции как основного источника сознания сопоставляется с подходом Лапланша к переносу, который заключается в том, что аналитик, осознавая свое «незнание», обеспечивает «пустоту», в которой аналитический процесс пациента может эволюционировать. Такое сочетание идей теоретически связывается с появляющимся в нейронауках пониманием восприятия; для иллюстрации обсуждаемого материала в статье делаются ссылки на клинический материал.
