Lumbar disc herniation has a high prevalence and strong social-medical impact. Patients suffer from lower back pain that radiates from the spine. Loss of sensation or paresis adds to the clinical picture. The diagnosis should be confirmed by imaging in patients considered for surgery. High remission rates initially warrant conservative treatment (adequate analgesia and physiotherapy) in many patients. If this treatment does not lead to significant alleviation within 5-8 weeks, surgery should be performed to reduce the risk of chronic nerve affection. Posterior interlaminar fenestration is the intervention primarily conducted for this diagnosis.A relapse in the same region occurs in up to 10% of patients after months through years, which sometimes necessitates a reoperation if symptoms are pertinent. La hernie discale lombaire constitue de par sa haute prévalence et ses conséquences socio-médicales un tableau clinique important. Celui-ci se compose de douleurs dorsales accompagnées d'une irradiation douloureuse, parfois de déficits sensitifs oumoteurs. Le diagnostic de présomption doit être confirmé par une imagerie appropriée, dans la mesure où une opération est envisagée. Un taux de rémission spontanée élevé justifie initialement dans de nombreux cas une thérapie conservatrice avec analgésie adéquate et physiothérapie. Dans les cas où cette prise en charge n'apporte pas d'amélioration significative dans un délai de 5 à 8 semaines, une opération est indiquée, afin de réduire le risque d'une atteinte nerveuse chronique. La procédure classique est alors une approche interlamaire postérieure sous microscope. Jusqu'à 10% des cas présente dans les mois ou années qui suivent une hernie localement récidivante, qui nécessite la plupart du temps une ré-opération si la symptomatique l'indique. Der lumbale Bandscheibenvorfall stellt durch seine hohe Prävalenz und sozialmedizinische Bedeutung ein wichtiges Krankheitsbild dar. Klinisch imponiert er durch Rückenschmerzen mit radikulärer Schmerzausstrahlung; gegebenenfalls auch Sensibilitätsausfällen oder Paresen. Die klinische Verdachtsdiagnose sollte durch eine geeignete Bildgebung überprüft werden, sofern eine Operation in Erwägung gezogen wird. Eine hohe Remissionsrate rechtfertigt in vielen Fällen eine initial konservative Therapie mit adäquater Analgesie und Physiotherapie. Führt dieses Vorgehen in 5 bis 8 Wochen nicht zu einer signifikanten Beschwerdelinderung, ist gegebenenfalls eine Operation indiziert, um das Risiko chronischer Nervenschädigungen zu reduzieren. Das Standardverfahren ist hierbei die posteriore interlaminäre Fensterung in mikrochirurgischer Technik. Nach Monaten bis Jahren kommt es in bis zu 10% der Fälle zu einem lokalen Rezidivvorfall, was bei entsprechender Symptomatik meist eine nochmalige Operation erfordert. [ABSTRACT FROM AUTHOR]
