Sensitivity of a Hymenoptera serological immunoglobulin (Ig)E assay for the diagnosis of venom hypersensitivity in dogs.

Background: Hymenoptera envenomation with honey bee (Apis mellifera) and paper wasp (Polistes spp.) may cause life‐threatening anaphylaxis in dogs. In human patients, clinical history, intradermal testing (IDT) and measurement of allergen‐specific serological immunoglobulin (Ig)E (sIgE) are used to support a diagnosis of Hymenoptera venom hypersensitivity. The utility of venom allergen‐specific sIgE has not yet been evaluated for this purpose in dogs. Objectives: The objective of the study was to investigate the sensitivity (sn), specificity (sp) and positive predictive value (PPV) of honey bee and paper wasp serological titres using a commercially available sIgE assay [VARL (Veterinary Allergen Reference Laboratory) Liquid Gold] against clinical history for a diagnosis of Hymenoptera hypersensitivity in dogs. Materials and Methods: Honeybee and paper wasp IgE serum titres were measured in 15 client‐owned dogs with a diagnosis of Hymenoptera venom hypersensitivity based on a history of anaphylaxis, owner observation of Hymenoptera, and positive IDT to bee and/or wasp; and in 10 client‐owned dogs with no known history of anaphylaxis or Hymenoptera exposure and a negative IDT to bee and wasp. Results: Analysis of receiver operating characteristic (ROC) curves demonstrate that a VARL score cut‐off of one of six for honeybee yields Sn, Sp and PPV of 40%, 60% and 60%, respectively, and two of six for wasp yields Sn, Sp and PPV of 25%, 78% and 60%, respectively. Conclusions and Clinical Relevance: Owing to the poor sensitivity and PPV of sIgE assays for both bee and wasp hypersensitivity in dogs with known envenomation and anaphylaxis, the use of sIgE cannot be recommended as a tool for venom identification. RESUMEN: Introducción: el envenenamiento por himenópteros con abejas melíferas (Apis mellifera) y avispas papeleras (Polistes spp.) puede causar anafilaxia potencialmente mortal en perros. En pacientes humanos, la historia clínica, las pruebas intradérmicas (IDT) y la medición de inmunoglobulina (Ig)E (sIgE) serológica específica de alérgeno se utilizan para respaldar un diagnóstico de hipersensibilidad al veneno de himenópteros. La utilidad de la sIgE específica para alérgenos de veneno aún no se ha evaluado para este propósito en perros. Objetivos: Investigar la sensibilidad (sn), la especificidad (sp) y el valor predictivo positivo (PPV) de los títulos serológicos de abejas melíferas y avispas papeleras utilizando un ensayo sIgE disponible en el mercado [VARL (Laboratorio Veterinario de Referencia de Alérgenos) Liquid Gold] en casos con un diagnóstico de hipersensibilidad a himenópteros en perros. Materiales y métodos: Se midieron los títulos séricos de IgE para abejas melíferas y avispas papeleras en 15 perros de propietarios particulares con un diagnóstico de hipersensibilidad al veneno de himenópteros basado en un historial de anafilaxia, observación de himenópteros por parte del propietario e IDT positivo para abejas y/o avispas; y en 10 perros de propietarios particulares sin antecedentes conocidos de anafilaxia ni exposición a himenópteros y una IDT negativa para abejas y avispas. Resultados: El análisis de las curvas de características operativas del receptor (ROC) demuestra que un valor de corte de VARL de uno de seis para abejas produce resultado de Sn, Sp y PPV de 40%, 60% y 60%, respectivamente, y un valor de corte de dos de seis para avispas resulta en Sn, Sp y PPV del 25%, 78% y 60%, respectivamente. Conclusiones y relevancia clínica: Debido a la escasa sensibilidad y PPV de los ensayos de sIgE para la hipersensibilidad de abejas y avispas en perros con envesnenamiento y anafilaxia conocidos, no se puede recomendar el uso de sIgE como herramienta para la identificación del veneno. Résumé: Contexte: L'envenimation par des hyménoptères tels que l'abeille domestique (Apis mellifera) et la guêpe (Polistes spp.) peut provoquer une anaphylaxie potentiellement mortelle chez les chiens. Chez les patients humains, l'anamnèse, les tests intradermiques (IDT) et la mesure des immunoglobulines (Ig) E sériques spécifiques de l'allergène (sIgE) sont utilisés pour étayer le diagnostic d'hypersensibilité au venin d'hyménoptères. L'utilité des sIgE spécifiques à l'allergène du venin n'a pas encore été évaluée à cette fin chez le chien. Objectifs: Étudier la sensibilité (sn), la spécificité (sp) et la valeur prédictive positive (PPV) des titres sériques relatifs à l'abeille et à la guêpe en utilisant un test sIgE disponible dans le commerce [VARL (Veterinary Allergen Reference Laboratory) Liquid Gold] en comparison à l'anamnèse pour un diagnostic d'hypersensibilité aux hyménoptères chez les chiens. Matériels et Méthodes: Les titres sériques d'IgE dirigés contre l'abeille et à la guêpe sont mesurés chez 15 chiens appartenant à des clients avec un diagnostic d'hypersensibilité au venin d'hyménoptère basé sur des antécédents d'anaphylaxie, l'observation d'hyménoptères par le propriétaire et un IDT positif à l'abeille et/ou à la guêpe ; et chez 10 chiens appartenant à des clients sans antécédents connus d'anaphylaxie ni d'exposition aux hyménoptères et avec un IDT négatif à l'abeille et à la guêpe. Résultats: L'analyse des courbes ROC (receiver operating characteristic) montre qu'un seuil de score VARL de un sur six pour l'abeille mellifère donne des Sn, Sp et PPV de 40%, 60% et 60%, respectivement, et deux sur six pour la guêpe donne des Sn, Sp et PPV de 25%, 78% et 60%, respectivement. Conclusions et pertinence clinique: En raison de la faible sensibilité et de la faible VPP des tests sIgE pour l'hypersensibilité aux abeilles et aux guêpes chez les chiens dont l'envenimation et l'anaphylaxie sont connues, l'utilisation des sIgE ne peut pas être recommandée comme outil d'identification du venin. Zusammenfassung: Hintergrund: Eine Hymenoptera Vergiftung durch die Honigbiene (Apis mellifera) und die Feldwespe (Polistes spp.) kann bei Hunden eine lebensbedrohliche Anaphylaxie auslösen. Bei menschlichen Patienten werden die klinische Anamnese, Intradermaltests (IDT) und die Messung allergen‐spezifischer serologischer Immunglobulin (Ig)E (sIgE) verwendet, um die Diagnose einer Hypersensibilität auf Hymenoptera Gifte zu bestätigen. Der Einsatz von Gift Allergen‐spezifischem sIgE zu diesem Zweck wurde bei Hunden bisher noch nicht evaluiert. Ziele: Um die Sensibilität (sn), Spezifität (sp) und den Positiven Vorhersagewert (PPV) von Honigbiene und Feldwespe zu untersuchen, sollten serologische Titer mittels kommerziell verfügbarem sIgE Assay [VARL (Veterinary Allergen Reference Laboratory) Liquid Gold] erfasst und der klinischen Anamnese zur Diagnose einer Hymenoptera Hypersensibilität bei Hunden gegenübergestellt werden. Materialien und Methoden: Es wurden die IgE Titer von Honigbiene und Feldwespe bei 15 Hunden in Privatbesitz mit der Diagnose einer Hymenoptera Gift Hypersensibilität basierend auf der Anamnese einer Anaphylaxie, BesitzerInnen Beobachtung von Hymenoptera und einem positiven IDT auf Biene und/oder Wespe gemessen; das gleiche wurde bei 10 Privathunden ohne die Anamnese einer Anaphylaxie oder einer Hymenoptera Exponierung und einem negativen Bienen und Wespen IDT gemacht. Ergebnisse: Die Analyse einer Grenzwertoptimierungskurve (ROC) zeigte, dass ein VARL‐Grenzwert von eins bis sechs für die Honigbiene eine Sn, Sp und ein PPV von 40%, 60% bzw 60% ergab, sowie ein Grenzwert von zwei bis sechs bei der Feldwespe eine Sn, Sp und ein PPV von 25%, 78% bzw 60%. Schlussfolgerungen und klinische Bedeutung: Aufgrund der geringen Sensibilität und des schlechten PPV‐Werts eines sIgE Assays sowohl für die Honigbiene wie auch für die Feldwespe beim Hund mit einer bekannten Vergiftung und Anaphylaxie, kann der Einsatz von sIgE als Mittel zur Giftidentifizierung nicht empfohlen werden. 要約: 背景: ミツバチ(Apis mellifera)およびアシナガバチ(Polistes spp.)によるハチ目中毒は、犬の生命を脅かすアナフィラキシーを引き起こすことがある。ヒト患者では、病歴、皮内テスト(IDT)およびアレルゲン特異的血清免疫グロブリン(Ig)E(sIgE)の測定が、ハチ毒過敏症の診断に用いられる。毒アレルゲン特異的sIgEの有用性は、犬ではまだ評価されていない。 目的: 本研究の目的は、犬におけるハチ目過敏症の診断のため市販sIgEアッセイ[VARL(Veterinary Allergen Reference Laboratory)Liquid Gold]を用いたミツバチおよびアシナガバチの血清学的力価の感度(sn)、特異度(sp)および陽性適中率(PPV)を臨床病歴と比較検討することであった。 材料と方法: アナフィラキシーの既往歴、飼い主によるヒメバチの観察、およびハチおよび/またはスズメバチに対するIDT陽性に基づき、ハチ目毒過敏症と診断されたオーナー所有犬15頭と、アナフィラキシーの既往歴もハチ目への曝露歴もなく、ハチおよびスズメバチに対するIDT陰性であったオーナー所有犬10頭について、ミツバチおよびアシナガバチのIgE血清力価を測定した。 結果: 受診者動作特性(ROC)曲線解析から、ミツバチに対するVARLスコアのカットオフ値を6段階中1段階にすると、Sn、Sp、PPVはそれぞれ40%、60%、60%となり、スズメバチに対するVARLスコアのカットオフ値を6段階中2段階にすると、Sn、Sp、PPVはそれぞれ25%、78%、60%となることが示された。 結論と臨床的意義: 既知の刺咬およびアナフィラキシーを有する犬におけるハチおよびスズメバチ過敏症のsIgEアッセイの感度とPPVが低いことから、sIgEの使用は毒同定ツールとしては推奨できない。 [ABSTRACT FROM AUTHOR]
Resumo: Contexto: Envenenamento por Hymenoptera causado por abelhas (Apis mellifera) e vespa de papel (Polistes spp.) pode causar anafilaxia fatal em cães. Em pacientes humanos, o histórico clínico, teste intradérmico (IDT) e mensuração de imunoglobulina (Ig)E (sIgE) alérgeno‐específica sérica são utilizados para corroborar com o diagnóstico de hipersensibilidade ao veneno de Hymenoptera. A utilidade da sIgE alérgeno‐específica para o veneno ainda não foi avaliada para este propósito em cães. Objetivos: Investigar a sensibilidade (sn), especificidade (sp) e valor preditivo positivo (VPP) de títulos sorológicos para abelhas e vespas de papel usando um ensaio sIgE comercialmente disponível [VARL (Veterinary Allergen Reference Laboratory) Liquid Gold] em animais com histórico clínico sugestivos de diagnóstico de hipersensibilidade a Hymenoptera em cães. Materiais e Métodos: Os títulos séricos de IgE de abelhas e vespas de papel foram medidos em 15 cães de clientes com diagnóstico de hipersensibilidade ao veneno de Hymenoptera com base em um histórico de anafilaxia, observação de Hymenoptera pelo proprietário e IDT positivo para abelha e/ou vespa; e em 10 cães de clientes sem histórico conhecido de anafilaxia nem exposição a Hymenoptera e IDT negativo para abelhas e vespas. Resultados: A análise das curvas da característica operacional do receptor (ROC) demonstrou que um ponto de corte VARL de um de seis para de abelhas fornece Sn, Sp e PPV de 40%, 60% e 60%, respectivamente, e dois de seis para de vespas forncece Sn, Sp e PPV de 25%, 78% e 60%, respectivamente.
